How it works

    Nationwide Direct Primary Care

Nationwide direct primary care is a flat monthly membership for primary care, urgent care and virtual visits that works in every state, not just at one clinic. Here's how FlexCare 365 works, what it costs and what it does and doesn't cover.

    How it works

    1. 1

      Join online

Pick a plan and enroll. You don't need insurance or a Social Security number. Your membership starts on the 1st of the following month.

    2. 2

Call the care line when you need care

Members book through one dedicated care line, staffed by bilingual medical assistants. There's no calling around to find a practice that's taking new patients.

    3. 3

See a doctor in person or virtually

You're booked with a participating physician or urgent care facility near wherever you are that day, usually same day or next day. Virtual visits are $0 on every plan.

    4. 4

      Your records follow you

Your health records go with you between visits and between states, so you're not starting from scratch every time.

    What's included

    • Unlimited in-person primary care
    • Urgent care
    • Unlimited virtual visits at $0
    • Help managing 13 ongoing conditions, including blood pressure, diabetes, asthma, anxiety, depression and thyroid disorders
    • An annual physical with basic labs after 6 consecutive months of membership
    • A $0 list of common short-term medications and $5 pricing on a list of 200 long-term medications (not available in Maine or Washington)

    What it doesn't cover

    • Emergency room visits
    • Hospital stays and surgery
    • Specialist care
    • Walk-in visits (all appointments are booked through the care line)

FlexCare 365 is built to sit alongside insurance, not replace it. Many members keep a plan for the big, unexpected costs and use their membership for everyday care. If you think you're having a medical emergency, call 911.

    Nationwide vs local vs virtual-only

All three get called "direct primary care" or "doctor subscriptions." They're not the same product.

    A good fit if you

    • Travel for work or split time between states
    • Can't get in to see a doctor without waiting weeks
    • Have a high deductible and pay full price for regular visits
    • Are self-employed, a 1099 worker or between jobs
    • Have an ongoing condition that needs regular check-ins

Probably not the right fit if you

    • Need regular specialist or hospital care
    • Are on Medicare, Medicaid or Tricare
    • Want something to replace insurance entirely

    Common questions

It's a primary care membership that works across the country instead of at one clinic. You pay a flat monthly fee and get access to primary care, urgent care and virtual visits through a network of participating physicians and urgent care facilities in all 50 states.

No. It covers everyday care: checkups, sick visits, urgent care, virtual visits and ongoing conditions. It doesn't cover the ER, hospital stays, surgery or specialists, and it doesn't count as health coverage under the Affordable Care Act. Many members keep an insurance plan for those bigger costs.

Roughly 13,000 providers, made up of board-certified physicians and urgent care facilities, with in-person access in all 50 states.

Once your membership is active, appointments are usually same day or next day, typically within 48 hours. Virtual visits are often faster.

Yes. That's the main difference from a local direct primary care practice. Your membership works in any state, through the same care line, with your records following you.

Plans are $167 a month for Individual Basic, $247 for Individual Premium, $277 for Family Basic and $449 for Family Premium, plus a one-time $50 enrollment fee. Basic plans have a $10 in-person visit fee and a $25 urgent care fee. Premium plans have no visit fees. Virtual visits are $0 on every plan.

No. You don't need insurance, an insurance card or a Social Security number. Memberships are open to ages 2 to 65. People on Medicare, Medicaid or Tricare aren't eligible.

On the 1st of the month after you join. It's built for ongoing care, so it's best set up before you need it.

See a doctor wherever you are

Join by the end of the month and your membership starts on the 1st.

FlexCare 365 is a healthcare membership, not insurance. It does not satisfy ACA minimum essential coverage requirements and is not a replacement for insurance.
